Per Guideline We Must verify that all sites participating in the program will  be proper stwereds for this amazing USDA program. Please answer below to the best of your abilities the following questions. Please now that this is NOT a graded assignment but will allow PCCPA staff to best understand the needs and desires for your organization. Please fill this out ONE per entity, regardless if you have numerous locations.
