Application and Enrollment IS HERE. This must be completed by Parent And Any additional Paper Copy.
Please note this is ONLY to be submitted for approved site types requesting reimbursement towards allowable expenses under your approved USDA plan.

Home Delivered Meals:
1. Only sites approved by the CE and submitted for waiver by TDA are approved to utilize this meal distribution method.
2. Children do not need to be present when using this meal distribution method. •
Note: If no one is present at the home, meals can be left, but make sure the Food Safety Best Practices page in this packet is reviewed and local health, safety, and sanitation standards are referenced. **unaffiliated sites are FULLY responsible for following all local health and safety standards.
• Include one “Food Safety Flyer” for each meal or a group of meals provided.
1. Make sure parents/guardians are aware of delivery schedule.
• Post schedule on website or use a phone service to contact participants.
2. Make sure to account for every delivered meal before leaving current location.
3. If anyone or anything, other than the meals, come in contact with the gloves, staff needs to remove and throw away gloves, wash/sanitize hands, and put new gloves on before next delivery.
4. Remember, all surfaces and vehicles carrying meals must be sanitized thoroughly before delivery method can begin.
